Fees and Insurance

You are responsible for the fees for the services I render to you at the time of the session. My fee for the initial session is $125. The fee for subsequent sessions is $100 per session (unless we have agreed on another rate).

I am a participating provider in several different insurance plans, under which I have agreed to accept a reduced fee for each session. If you are covered by an insurance plan in which I participate, your insurance company can give you the details on your financial obligations. If I am not a provider on your plan, they may, nonetheless, provide benefits for your therapy if you have out-of-network benefits. These out-of-network benefits differ somewhat from in-network benefits. You may have a higher deductible or co-pay when you see an out-of-network provider. Please consult with your insurance company about your benefits.

I will submit claims to your insurance company for you, however, if your insurance refuses to cover the service, you are responsible for the charges for your therapy. Additionally, most people are responsible for a copayment or for a deductible before their insurance will provide any coverage. This co-pay or deductible is your financial obligation. You are responsible for making this payment on the day of your appointment. If you are uncertain about whether you have met your deductible for the year, I ask that you pay the charge for the session, at the rate that your insurance company allows, on the day of the session. If we don’t know how much your co-pay is, I ask that you make payments of $25 per session toward your co-pay until we find out the exact amount. Of course, if we find that you have overpaid on your deductible or your copay, I will refund this overpayment to you. For telephone conversations which are longer than ten minutes, I will bill you at the rate of $100 per hour (unless we have agreed on a different rate). Please be aware that your insurance company will not cover the expenses of phone consultations. Clients who do not satisfy their portion of the bills for their therapy are responsible for the court costs and attorney fees if I have to go to court for a judgment to collect those debts.

Your insurance company may require pre-authorization for you to come to therapy. I ask that, before our first session, you contact your insurance company and, if required, get authorization for them provide benefits for your therapy. They will give you an authorization number. Please bring that number with you to your session or send me an email with that number as well as any other information they may have given you.

Your insurance company may authorize a limited number of sessions initially. If your work in therapy requires more sessions, I will contact your insurance company for additional authorizations.

Your insurance company may need you to provide them with information in order to authorize sessions or process claims. If you receive any correspondence from your insurance company about your therapy, please bring it in and we will discuss it.
